New Patient Information

For new patients we ask that you arrive 45 minutes prior to your appointment to complete the registration packet. If you would prefer to not come as early, you are welcome to print off a PDF copy of our registration packet and fill it out beforehand. Please see the instructions in the sections below on how to complete your registration paperwork at your leisure. Your registration paperwork is an important part of your first appointment. If the registration paperwork is not complete before your appointment then you run the risk of not being able to be seen, along with a fee for rescheduling

Completing the New Patient Packet at Home
If you choose to complete the registration before your appointment please review the following items.

  • Please be sure to fill out the packet in blue or black ink only.
  • Do not duplex print. Please only print on one side of a page.
  • All authorization sections need to be completed prior to being seen.

General Appointment Reminders

  • Your insurance card and a form of identification are necessary for your initial appointment and follow up appointments.
  • The only suitable form of identification is that which is issued by the government.
  • Make sure to verify your benefits including any deductible, copay, or coinsurance.
  • If you need to cancel or reschedule your appointment please make sure to give at least 48 business hours notice.
  • Please be early to your appointment as late arrival may result in not being seen and a fee to reschedule.

Patient Portal

By registering for the patient portal you will be able to view demographic information, access financial information, get scheduling information, and much more.

Registration is by invite only, contact our office if you would like to connect with the portal. New features are being planned for the patient portal, updates on when they’re available will be shared here on our website and in our office.

Our patient portal vendor is CareCloud/Breeze, communications from the portal may come through as Breeze and/or CareCloud depending on how you choose to access it (website compared to mobile app).
